How Great Lakes PACE Compares to MI Choice Waiver Program

Great Lakes PACEMedicaid Waiver
Purpose?Provides comprehensive, all-in-one care to keep seniors living safely in their homes and communities.Provides specific services to help people avoid institutional care (like nursing homes).
Funding?Jointly funded by Medicare and Medicaid (and private pay if not Medicaid-eligible).Funded entirely by Medicaid.

One of our caring Intake Coordinators will contact you to collect all of the necessary information to get you enrolled. Then we’ll schedule a Home Visit to meet you in person and to help you determine what kinds of services would be most beneficial for you. Family is welcome to be involved in the process.
