We know how lonely and challenging it can be as a caregiver. What you have chosen to do demands dedication, physical stamina, and compassion. Our objective is to support you by providing practical, meaningful services that are included for all Great Lakes PACE participants.
- Day Center Services. When someone is accepted into the Great Lakes PACE program, they’re welcome at our Day Center Monday through Friday during business hours. At no additional charge, they’ll have access to individual and group activities as well as meals.
- Home Care Services. We can assist with meal prep, personal care, house work, Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) needs.
- Transportation to Great Lakes PACE. Door to door transportation service to Great Lakes PACE. We’ll help them from their home, into our accessible vehicle, into our Day Center, and back home again.
- Transportation to Appointments. Door to door transportation service to medical appointments. We’ll help them from their home, into our accessible vehicle, into their appointment, and back home again.
- Respite Care. If you’re scheduling a vacation, business trip, or just need a few days to rest or focus on tasks, we can provide respite care at one of our contracted facilities within our community.
- Caregiver Education. One on one education provided to caregivers by the Great Lakes PACE medical team. How can we help and best support you?
- Free Consultations. Contact us for your free consultation where you can learn details about numerous resources available to you and your loved one.
