Can you participate in PACE?

  • Are you 55 years of age or older?
  • Are you eligible for nursing home level of care as determined by the State of Michigan? We can help you determine your eligibility for nursing home level of care.
  • Are you able to live safely at home with the support of Great Lakes PACE? We can help you determine your current level of safety.
  • Are you residing in the Great Lakes PACE service area? Service area includes these counties: Saginaw, Bay, Midland, Tuscola, Gratiot, Arenac, Shiawassee.

Can you afford PACE?

Your cost will depend on your Medicaid and Medicare eligibility:

  • Are you eligible for both Medicaid and Medicare? There is no out-of-pocket cost. We can help you determine your eligibility.
  • Are you only eligible for Medicare? There is a monthly fee.

Because Great Lakes PACE provides and is responsible for all of your care, you may be financially responsible for any care you receive outside the program that is not approved by the Great Lakes PACE Interdisciplinary Team (except in an emergency).
